Preparing for a Visit

If you wish to make an appointment: Please call (516) 827-0770.

The first visit to our facility will include a comprehensive assessment of your wound. Initial assesments generally take forty-five minutes to an hour to complete. During this visit, you may have blood drawn that will provide us with information that helps us for a treatment plan. It is important that you provide us with a complete and accurate history of your wound and treatment to date.

Please make sure to bring:

  • A list of current medications and dosages
  • A list of allergies
  • A list of questions which you may have
  • Location and phone number of your pharmacy
  • Insurance Cards and referral authorization, if necessary

If you are unable to perform your own wound care for any reason, please ask the person(s) that will be assisting you in your wound care to accompany you to your initial visit. We will instruct you and your caregiver in the proper technique at your initial visit.

Even though our office is handicapped accessible, if you need assistance transferring from a wheelchair, you will need to bring someone that is capable of assisting you in your transfer to our power tables. This may be the same person who will be assisting you in your wound care.

If possible, please wear comfortable, loose-fitting clothing and "step-in" shoes.
